POLICIES

GROUP SESSIONS:
BEGINNER U4-U7 GROUP SESSION: We offer this on Tuesdays.
DEVELOPMENT U8-U13 GROUP SESSION: We offer this group Monday, Wednesday.
ADVANCED 13+: We offer this class Tuesday, Thursday
HIGH SCHOOL/ COLLEGE GROUP SESSION: This session is for high level players, including college athletes and those preparing for the next level. We offer this Friday nights.
PRIVATES SESSIONS: ALL AGES. This is offered Monday, Wednesday, Thursday, Friday, Saturday.
SHOOTING CLINICS: This is offered Saturday mornings at 9am. Our shooting clinic is designed for players of all skill levels who want to improve their shooting mechanics, body positioning, accuracy, and striking technique. Players will work through a variety of finishing patterns, first-touch preparation, and decision-making in the final third.
Training groups may be adjusted based on age and ability to ensure every player is challenged appropriately, but all players will participate in the same clinic.

Our packages are designed to make trainings simple and consistent.
Each package includes 4 sessions. You can use these sessions for any available days/time that matches your appointment type.
After purchasing package, click “USE PACKAGE” and it will return you to the booking page and choose the dates that work best for you.
Spots are first-come, first-served. Booking early helps you secure the days you want.

You can book all of your sessions as early in advance as you’d like, or if needing more flexibility, you can book as late as 6 hours before session start time. This 6 hour rule applies to all services. Once it’s less than 6 hours before session start time, the class or appointment time will disappear from the schedule and you will be unavailable to book that slot.
This 6 hour rule also applies to cancellations and reschedules. If you have booked yourself for a session, any changes to availability must be made before the 6 hour mark.
RESCHEDULING & CANCELLING SESSIONS:
When booking a session, you will automatically get a confirmation email or text.
Inside the message there is a “Change/Cancel Appointment” Link.

Reschedule to another available time.
Cancel the session

If you are more than 15 minutes late to your appointment time, it will be a lost session.

All Payments must be submitted when scheduling session(s) through Acuity.
We DO NOT accept cash payments, Zelle, checks, or any other form of payment.

We have a no refund policy. We will also not transfer credits or funds to different packages.
Extensions
All memberships and packages have specific expiration dates listed when you buy your pricing plan. We will NOT extend package expirations. Unused sessions in memberships and packages will NOT rollover to the next billing period.

SINGLE GROUP SESSION - $40 (30 day expiration)
4 PACK GROUP SESSIONS - $120 (30 day expiration)
PRIVATE SESSION PACKAGES:
Single Private Session - $60 (30 day expiration)
4 PACK PRIVATE SESSIONS - $ 180 (30 day expiration)
SHOOTING CLINIC PACKAGES:
SINGLE SHOOTING CLINIC - $30 (30 day expiration)
4 PACK SHOOTING CLINICS - $100 (30day expiration)

During inclement weather, we will make effort to schedule a makeup session on the following weekend.
Players are also welcomed to make up missed sessions the following week on any day their session is offered.

All Forma sessions are recorded for the use on social media and other marketing platforms. By participating in any Forma Futbol Academy events, you are consenting to the use of all footage and photos to be used in any and all marketing platforms for Forma Futbol Academy.

Players will receive one free group session if their referral signs up for a package or membership.

Forma players are held to a high standard of expectations. All players must come with an entrenched level of focus and ambition. We only take on players that are ready to fully commit in reaching their goals. Players that slack off or distract the group from learning will be introduced to our two-strike policy. After two warnings, players may be asked to sit out for the remainder of the drill or session. We aim to keep a very fast paced learning environment and want to give every player the best opportunity to reach their goals.
Players also need to come in appropriate training gear for practice. No running shoes.
Younger players need to have their shoelaces double knotted and their hair pulled back out of their face.

We ask all spectators (parents, siblings, and friends) to respect our no distraction policy. Please make sure all comments and criticism towards players are held until after the session is over. We understand your enthusiasm, but we want to keep the player(s) focused on the task at hand. We highly encourage all parents to stay and observe our sessions, but do ask you to stay on the outskirts of the training grounds.

Forma will occasionally send out important mobile announcements mostly regarding changes due to weather, cancellations, or policy updates.
